How fees work
There is one fee conversation, before you begin, and it is in writing.
Insurance we work with
Salve is in network with some health plans, and which plans a clinician takes can differ. We check yours before your first session and tell you what we found. We do not take every plan, and we say so plainly.
Most PPO plans
Some HMO plans
Medicare and Medicaid: ask us, and we will say plainly whether we take yours
If you pay yourself
If you pay without insurance, your fee depends on the clinician and the length of the session. Most sessions are 50 minutes. A limited number of reduced-fee spots are held, so ask when you write. Your fee is given in writing before you start. You can pay with a health savings or flexible spending card. When you pay yourself, no claim goes to a plan.
Receipts for reimbursement
On request we give you a monthly itemized receipt (a superbill) that you can send to your plan to ask for reimbursement. It carries a diagnosis code that your plan sees, and a plan that pays you back may send a statement to the person who holds the policy. Your plan decides what it pays.
An estimate in advance
If you pay without insurance, you have the right to receive a Good Faith Estimate of the cost of your care before you start. We give you one in writing. If a bill is $400 or more above the estimate, you can dispute it. The estimate is not a contract.
Checking your benefits
Before your first session we check your benefits and tell you what we found, including any copay or deductible your plan sets. You hear from us by call, text or email, whichever you chose. If your plan comes through your employer, we explain how its statements are sent before you decide.
Before your first session
What to have ready
Your plan, if you use one
The plan name and member number, or a photo of the card. Tell us if the plan comes through your employer.
Your questions
Ask what each session would cost you, what is not covered and who gets a statement. We answer plainly.
Your choice
You can switch from insurance to paying yourself before any session. Tell us and we give you a new estimate.
